AZSC is a program of not-for-profit Hospice of the Valley, serving central Arizona since 1977. Unlike other palliative care programs operated by multistate corporations, AZSC is based locally, hires locally and has longstanding relationships with our community.
Care is provided through partnerships with contracted health insurance plans, and we encourage you to contact us if you need assistance with symptom management, coordination of care or access to health-support services. Even if the insurance provider isn’t a contracted partner, we have other supportive care programs that can help.
Our top priority is to help you stay safe and healthy in your own home. We want to improve your quality of life, whether it’s ensuring a smooth transition back home after a hospital stay or managing your condition to avoid emergency room visits and hospitalizations. This company done exactly what they said they were going to do. The social worker, NP, nurse, all called ASAP and made appointments to come out and see us. Everyone showed genuine concern for our well being. They turned us on to services we didn't know existed like Dispatch Health and an X-Ray Company that come to your house. Arizona Supportive Care has been such an amazing company to work for and the reason behind that is because of their amazing staff , I want to give a million thanks for Veronica and Benita who have assisted my uncle and I in ever way possible. Our social worker helped us from start to finish. My partner is now my full-time caregiver and I am getting the care I need. We now have someone we can call for answers to questions we have. We find this support invaluable. We are so thankful Arizona Supportive Care has a program that is free and accessible to all people going through medical difficulties. Even my primary care doctor was impressed that I was able to get all the help I now have and he even did more things to help me. He told me, "Most people do not make it through this process."
