Arizona Supportive Care (AZSC) is a home-based program that comes to you on your schedule. Our team of medical professionals collaborates with your physician to provide services in the comfort of home wherever you live — your house, an assisted living community or a skilled nursing facility.
You may receive this comprehensive care while continuing to pursue other curative medical treatments with your own doctor. And it’s available no matter what stage of chronic illness you have.
Our AZSC physicians, nurses, nursing assistants, chaplains and social workers provide compassionate, specialized support — not just to patients, but also to overwhelmed family caregivers. We explain medications and disease progression, coordinate care with specialists, refer to community resources and assist with medical directives.
In other words, we are always there for you, from simple questions to urgent needs. Our 24/7 phone line is answered by a nurse — never an answering service — nights, weekends and holidays included.
Our top priority is to help you stay safe and healthy in your own home. We want to improve your quality of life, whether it’s ensuring a smooth transition back home after a hospital stay or managing your condition to avoid emergency room visits and hospitalizations.
